What the inspector wrote

On February 9, 2026, Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of conducting the Required- 1 Year evaluation using the Care Inspection Tool. LPA was greeted and granted entry after explaining the reason for the visit to Caregiver John Montolalu. Administrator Maryam Alamoutinia arrived on site and remained at the facility for the duration of the evaluation. The administrator's certificate for Maryam expires on February 3, 2026, and all course work has been completed. The inspection is as follows: This facility is a single story residential home comprised of six private resident bedrooms, five resident bathrooms, and one shared staff bedroom. The resident census is six (6) and there are two (2) staff on duty. Facility is operating within the conditions and limitations specified on the license. LPA toured the premise accompanied by Caregiver Aina Tsou. LPA observed all resident bedrooms had the required elements with ample lighting. All bathrooms were clean and operational. The hot water temperature measured at 110.8, 109.2, 113.1, 112.4, and 111.9 degrees Fahrenheit. There were a clean supply of linens. LPA observed ample two-day supply of perishables and seven-day supply of non-perishable food. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed the fire extinguisher was mounted, charged, and serviced on April 11, 2025. The indoor temperature remains at a comfortable range. The smoke/carbon monoxide detectors were tested and operational. The fire place was screened, and there are no bodies of water on premise. Facility maintains emergency food, water, and disaster supplies as noted on the Emergency Disaster Plan including the first aid kit. LPA reviewed six of six resident files and two of two personnel files in which no discrepancies were found. Medications were audited for two of six residents, and no discrepancies were found. Annual staff training is currently in progress with completion of more than 50%. Present staff are First Aid/CPR certified, background cleared, and associated to the facility. Disaster drills are conducted on a quarterly basis. The Complaint Poster in the correct size is posted in the entry way. Based on the observations made during today's visit, no deficiency is being cited. An exit interview was conducted with Administrator Maryam Alamoutinia, and a copy of this report was provided at exit.
