What the inspector wrote

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced case management visit to amend complaint control number 22-AS-20230728162610. Upon arrival, LPA Haddadin was greeted and granted entry by staff. The Licensee, Jay Shah, arrived later during the visit, and LPA Haddadin explained the purpose of the visit. An exit interview was conducted, and a copy of this report was provided to the Licensee, Jay Shah.
