What the inspector wrote

Facility failed to provide proper 30 day eviction procedures and did not include the necessary information and does not meet the requirements per Title 22 regulations. The preponderance of evidence standard has been met, therefore the allegation illegal eviction are deemed SUBSTANTIATED. California Code of Regulations Title 22 Division 6 chapter 8 is being cited on attached LIC 9099D. An exit interview was conducted and copy of Appeals rights and confidential names list was left at facility.

This requirement is not met as evidenced by facility did not comply with proper eviction procedures which poses an immediate health, safety or personal rights risk to persons in care.
