On May 7, 2026, Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of conducting a collateral to interview and obtain records for Resident #1 (R1) in connection to an investigation at another licensed facility for complaint control number: 22-AS-20260430132811. LPA was greeted and granted entry by Caregiver Blanca Gonzalez who then advised Administrator's Assistant Mark Realis by telephone the reason for the visit. During today's visit, LPA interviewed R1 by telephone and obtained the following records: Photo Identification, Identification and Emergency Information, and page 1 of the Admission Agreement. An exit interview was conducted with Caregiver Blanca Gonzalez authorized by Assistant Administrator Mark Realis, and a copy of this report was provided via email at exit.
