Based on record review, the licensee did not comply with the section cited above in 5 out of staff files which poses a potential health, safety or personal rights risk to persons in care.

Based on a review of Resident 1's medication and medication records, Resident 1 was missing 2 out of their 12 prescribed medications (Resident 1, was missing their MiraLax Packet 17 GM and Oxycodone HCI tablet 5 MG), the licensee did not comply with the section cited above in 1 out of 10 resident medications and records which poses an immediate health, safety or personal rights risk to persons in care.

Incidental Medical and Dental Care. Once ordered by the physician the medication is given according to the physician's directions. This requirement is not met as evidenced by: based on a review of records R1 was not administered any medications from 4/22/23 to 4/26/23 and on 4/30/23. This poses an immediate risk to the health & safety of residents in care.

Diabetes-The licensee shall be permitted to accept or retain a resident who has diabetes if the resident is able to perform his/her own glucose testing with blood or urine specimens, and is able to administer his/her own medication including medication administered orally or through injection, or has it administered by an appropriately skilled professional. This requirement, is not being met as evidenced by, evidence gathered in interviews revealed that staff (non-licensed) administer prescribed medication (insulin) through injections and perform glucose testing via lancet on residents without assistance. This poses an immediate health and safety risk to residents in care.

Basic services shall at a minimum include: Care and supervision as defined in Section 87101(c)(3) and Health and Safety Codesection 1569.2(c). This requirement is not being met as evidenced by: Based on interview and record review, Licensee did not ensure care and supervision was provided to Resident 1. R1 eloped from the facility on 9/15/2021 because staff did not respond to the door alarm. This poses an immediate health and safety risk to residents in care.
