What the inspector wrote

On November 21, 2025, Licensing Program Analyst (LPA) Eboni Bentley conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Bentley introduced self and was granted entry into the facility by staff, after stating the purpose of the visit. Licensee (LI) Brian Estorba arrived at the facility and remained to assist during the visit. The facility is licensed to operate for six (6) non-ambulatory residents of which one (1) may be bedridden, with a hospice waiver for five (5) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: six (6) resident bedrooms, three (3) staff bedrooms, six (6) bathrooms, living area, dining area, kitchen, an outdoor covered seating areas, and an attached two car garage. During the visit, LPA toured inside and outside of the physical plant with LI Estorba There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be clean and operational. Medications and sharps objects were observed unlocked and accessible to residents in care and a deficiency was cited. The water temperature in five resident bathrooms measured between 105.4 degrees F to 116.6 degrees F. A comfortable temperature of 74 degrees F was maintained in the facility. LPA observed the facility to be sanitary and appropriately furnished at the time of visit. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available and maintained properly. CONTINUE TO LIC809-C...... The facility has two (2) fire extinguishers that were charged and serviced on September 19, 2025. Emergency food, emergency water, and emergency supplies were observed. A working telephone (657)259-0700 remains available, however the facility does not currently have a device that can be used for video teleconference purposes. LPA provided Technical Assistance. The smoke detectors and carbon monoxide detectors were operable. First aid kit is maintained and contains all the necessary elements. Emergency safety drills were last conducted on April 10, 2025 and no record of current Liability Insurance was provided during the visit. Deficiencies were cited. LPA conducted an audit of five (5) resident files (R1-R5), five (5) staff files (S1-S5), and medication and medication administration review. Resident and staff interviews were conducted. Deficiencies were cited during this visit, as per Title 22 Division 6 Chapter 8 of the California Code of Regulations. An exit interview was conducted, and a copy of this report, LIC809-D, Technical Violation, Technical Assistance, and appeal rights were provided to Licensee Brian Estorba at exit.

LPA observed the Licensee does not currently have liability insurance for the facility. LPA observed a proposal for liability insurance dated June 17, 2025 and no additional records were provided during the visit.

    Based on observation and record review, the licensee did not ensure the facility staff received fire, emergency, and disaster training quarterly, as the last training was conducted on April 10, 2025. This poses a potential safety risk to persons in care.

    Based on observation and record review, the licensee did not comply with the section cited above. LPA observed unsecured scissors and medication in R1's bedroom. R1 has a diagnosis of dementia. Sharps were also observed unlocked in a cabinet in the kitchen, which poses an immediate risk to residents in care.

    Based on a record review Staff 3 (S3) does not have a transfer of their criminal record as required by the Department. This poses an immediate health, safety and personal rights risk to residents in care. Licensee and staff stated S3 has worked at facility for more than 3 years. CIVIL PENALTY ASSESSED.

    Based on observation, licensee did not comply with the section cited above as two black roaches were observed on the wall in the kitchen. This poses an immediate risk to persons in care.
