What the inspector wrote

On June 9, 2025, Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual evaluation at the facility. Care staff member Larawan Barnard granted LPA Haddadin entry. The facility is licensed for six (6) non-ambulatory residents and holds a hospice waiver for six (6) residents. It is a single-story structure located in a residential neighborhood and consists of the following: five resident bedrooms, one staff bedroom, two bathrooms, a living room, a dining room, a kitchen, an attached garage, and a covered outdoor patio area. During the visit, the LPA toured the facility and inspected the physical premises. This inspection included testing all smoke detectors, checking the hot water temperature in both resident bathrooms, and verifying the functionality of auditory devices on all exits. The hot water temperature measured between 113.3°F and 117.8°F, and all smoke detectors were operational. The fire extinguisher was fully charged and had been serviced on March 6, 2025. The facility's most recent disaster drill was conducted on March 4, 2025. The LPA inspected the food supply and confirmed that the facility maintained the required minimum of a two-day supply of perishable food and a seven-day supply of non-perishable food. The LPA also observed the medication storage area and reviewed the centrally stored medications, which appear to be administered as prescribed. All cleaning supplies, chemicals, and sharps were locked in kitchen cabinets and inaccessible to residents. A review of two of the four staff files, including training and fingerprint records, was conducted. The LPA also performed a complete review of resident records. Based on the observations made during this visit, the facility appears to be in compliance with the California Code of Regulations, Title 22, Division 6. No deficiencies were cited on this date. An exit interview was conducted with Larawan Barnard, and a copy of this report was provided.
