On April 27, 2026, Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. made an unannounced visit to the facility to conduct the required annual inspection. Upon arrival, LPA observed that there were no residents in care. LPA called Administrator (AD) Ryan Fisk who reported that there are no residents in care. AD Fisk reported that AD Brigette Fisk was on her way. AD Brigette arrived shortly after. For today’s visit, LPA observed a total of zero residents in care, LPA observed the Administrator's Certificate for facility AD Brigette Fisk which expires on September, 27, 2027. LPA and Fisk conducted a tour of the facility and observed the following: There are no residents in care. The facility is not providing care and/or supervision. LPA tour the inside and outside of the facility and observed that the grounds were free of any hazards. Per AD, the facility will remain open until further notice. AD was reminded to notify Licensing if they plan on closing and/or relocating the facility. There are no immediately health and safety concerns, as the facility has a census of zero. An exit interview was conducted with AD Fisk and a copy of this report was provided at the time of exit.
