What the inspector wrote

Licensing Program Analyst (LPA) Alvaro Ramirez, Jr made an unannounced case management visit in conjunction with Unusual Incident/Injury Report (UIIR) dated August 11, 2025. LPA was greeted and granted entry by Executive Director (ED) Vanessa Valencia. LPA explained the reason for the visit. Per UIIR on July 30, 2025, Resident's 1 (R1's) Primary Care Physician (PCP) had discontinued the medication Aripiprazole 15 mg and replaced it with a new order of Aripiprazole 10 mg to be administered once daily in the morning. Per UIIR on the mornings of August 1, 2025, August 2, 2025, August 3, 2025, August 4, 2025, and August 5, 2025, R1 was given both Aripiprazole 15 mg and Aripiprazole 10 mg. During today's visit LPA reviewed the Physician's orders dated August 2025, for R1. Per Physician's orders as of July 31, 2025, R1 was prescribed Aripiprazole 10 mg, 1 tablet by mouth daily. A Health and Safety inspection was conducted, and LPA Ramirez observed no Health and Safety concerns during today's visit. Based on observations and records reviewed during today’s inspection, one deficiency is being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with ED Valencia, a copy of Appeal Rights and a copy of this report was provided at the time of exit.

This requirement was not met as evidence by: R1 was given both Aripiprazole 15 mg and 10 mg from 8/1/25 to 8/5/25; however effective 7/31/25 R1 was only prescribed Aripiprazole 10 mg.
