Correction check·May 4, 2026

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct a proof of correction visit to follow up on the citations issued during the annual inspection visit on April 3, 2026. LPA was greeted and granted entry by staff. LPA explained the reason for the visit. LPA observed the fire extinguisher in the living room is fully charged and the tag is dated April 7, 2026. LPA measured the hot water in two bathrooms, the hot water measured 114.0 degrees Fahrenheit in both bathrooms. LPA observed S1 now has 4 hours of training for postural supports, restricted health conditions. LPA observed R3 now has a new Appraisal/Needs and Service Plan. All 4 citations have been cleared. No deficiencies observed during the visit. No deficiencies are being cited as a result of this visit. An exit interview was conducted and a copy of the report provided.
