What the inspector wrote

On this day, Licensing Program Analysts (LPAs) Kevin Saborit-Guasch and Hanna Gough made an unannounced visit to the facility for the purpose of conducting a case management inspection documenting a deficiency observed during the investigation of complaint reference 22-AS-20241231115747. During the initial complaint investigation, facility staff was unable to provide proof of background clearance for staff member S1, who was stated to have been recently hired to work at the facility. S1 could also not be located in Guardian by licensing staff. As a result, a type A deficiency is being cited along with an immediate civil penalty. An exit interview was conducted and a copy of this report along with appeal rights was provided to a facility representative.
