Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit visit to the facility to conduct a Case Management inspection. LPA was greeted and granted entry into the facility by Administrator (AD) Maryln Salazar. LPA observed that Administrator Marlyn Salazar has a valid Administrator's certificate which expires on May 15, 2025. The facility received a new Fire Clearance for an increase in capacity on April 21, 2025 from the Orange Fire Department. Orange Fire Department Inspector Robert Desihome Jr approved the facility to have four residents, of which two may be non-ambulatory and one may be bedridden in bedroom number three. On today's visit, LPA accompanied by the AD, conducted a tour of the facility. LPA observed the three residents rooms that were approved from the fire clearance are free of hazards. The three resident bedrooms have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. Based on the observations made during today's visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. An exit interview was conducted with Administrator Marlyn Salazar and a copy of the report was provided.
