Routine inspection (multi-day)·June 25, 2025

On June 25, 2025, at 8:00 AM, Licensing Program Analyst (LPA) Edward Kim conducted a continuation visit for a required 1-year annual visit. LPA Kim was greeted and gained entry to the facility by staff. Administrator Hany Fawaz arrived around 8:20 AM. LPA Kim conducted a record review on six staff files (S1-S6). An internet access device dedicated to the residents was available. The facility has smoke detectors and carbon monoxide detectors that were operable. First Aid Kit contained all the necessary elements. LPA Kim conducted interviews for one resident and two staff. LPA Kim reviewed resident medications and medication administration record. No deficiencies were cited during this inspection visit. An exit interview was conducted and a copy of this report provided to Administrator Hany Fawaz.
