Inspector’s narrative

What the inspector wrote

The two staff members added resident 1 (R1) is hard of hearing and must speak to them loudly to communicate. The remaining staff stated S1 yells but believes that is part of their culture. LPA attempted to interview R1 but was unable to as R1 is hard of hearing and visual cues were difficult for R1 to read. LPA record review indicated training for two staff members were up to date including mandated reporter and personal rights training. LPA observed R1 speaking in a loud voice when speaking to staff. LPA toured the facility and did not observe any health or safety concerns. Therefore based on resident interviews, staff interviews, records observed, and LPA observations, the allegation of staff member yells at residents in care is deemed unsubstantiated meaning that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. An exit interview was conducted and a copy of the report was left with the facility representative. LPA observed residents being attended to during the visit including breakfast being served and a visit by hospice personnel. LPA toured the facility and did not observe any health and safety concerns. Based on record review, interviews and observations, LPA is unable to corroborate the allegation. Therefore, the allegation is deemed to be UNFOUNDED, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. An exit interview was conducted and a copy of the report was left with the facility representative.
