LPA Haley attempted to conduct the required Annual Inspection. When LPA arrived the door bell was rang and there was no answer. LPA knocked on the door and there was no answer. LPA contacted the numbers on the facility profile. The mobile number was answered by Kevin Dinh who said he is not the licensee, he's only helping out. LPA then called the primary number on the profile and spoke with Licensee Andre Bui. Licensee Bui said he was out of town and not available to come to the facility for the inspection. Bui also stated there are no residents in the home and the facility has never accepted any residents as he is waiting on the ALW program. LPA request an email form the licensee following the phone conversation.
