This requirement is not met as evidenced by: Based on record review and interviews conducted, Licensee failed to ensure care and supervision was provided to resident. R1 eloped two times out of the facility on March 18 and 24, 2026 which poses an immediate health and safety risk to residents in care. CIVIL PENALTY ASSESSED
