What the inspector wrote

Licensing Program Analyst (LPA) Celine Rodriguez conducted an unannounced case management visit to the facility for the purpose of a health and safety check. During the visit, staff on duty contacted facility administrator (AD) Bhonalyn Ladia about visit. For this visit, there are two staff members on duty, of who were background cleared and associated, and LPA Rodriguez met with AD Ladia. For this visit, there are a total of 4 residents in care, of which 0 are on hospice and 0 bedridden. LPA Rodriguez reviewed and observed the following: Payroll records (August 2025-October 2025) - LPA Rodriguez obtained copies of payroll for staff. Food supply (August 2025-October 2025) - LPA Rodriguez observed that facility has 2-day perishable, 7-day non-perishable, and emergency food supply items. LPA verified that groceries are completed once a week. LPA Rodriguez obtained copies of grocery receipts. Utilities (August 2025-October 2025) - LPA Rodriguez conducted a tour of the facility and verified that utilities (gas, water, electricity) are operational. LPA tested water temperatures, of which were measured to be at 118.4 degrees Fahrenheit. Rent (August 2025-October 2025) - LPA Rodriguez obtained copies of rent payments. For today's visit no deficiencies were issued per Title 22 Division 6 of the California Code of Regulations. No citations were issued. No health and safety concerns noted. An exit interview was conducted with AD Ladia. A copy of this report was explained and provided.
