What the inspector wrote

Licensing Program Analyst (LPA) Eboni Bentley conducted an announced subsequent Pre-Licensing continuation visit. LPA Bentley was allowed entry and met with Applicant/Chief Executive Officer (CEO) Tin Le and family member Brian Canseco. The purpose of today's visit was to follow-up on the issues that were present during the initial Pre-Licensing visit on June 19, 2025. The following issues below were observed requiring correction: · All passageways (soil/hose) in the yard were cleared. · Grab bars in the resident bathrooms were installed. · Land line phone service, (714) 929-1223, was tested and remains available. · All medications, sharps (gardening tools, scissors), and toxins (cleaning solutions) were inaccessible and secured. · The home/resident’s bedroom spaces were de-cluttered and ready for use. · All emergency supplies which include but are not limited to such as whistles were obtained per the Emergency Disaster Plan (LIC610E) · The LIC610E was amended and reflects the type of residents served. · The following postings were observed: food menu, activity calendar, sign in and out area, and admission agreement. CONTINUE TO LIC809-C .... · Facility does not currently have liability insurance. A proposal for liability insurance protection was observed. Component III is waived due to the applicant fulfilling the requirements previously for existing facilities. On today's visit the aforementioned items have been addressed and corrected. The aforementioned items reviewed during this visit are in compliance. The Pre-Licensing is now complete. The license will be granted upon completion of a final review and approval from the Licensing Program Manager and the Central Applications Bureau. An exit interview was conducted with the above individuals, and a copy of this report was provided at the time of this visit.
