What the inspector wrote

Licensing Program Analyst (LPA) Samer Haddadin made an unannounced visit to the facility to conduct an annual inspection. Upon arrival, LPA was greeted by and granted entry into the facility by Administrator (AD) Alexandra Gavrilas. The facility is a one-story home with five bedrooms and four restrooms. The facility has an approved fire clearance for five ambulatory residents and one non-ambulatory resident. At that time of this visit the facility had six residents in care. During the tour of the premises, LPA reviewed the storage of medications and confirmed they were properly labeled and maintained in a manner that was inaccessible to residents. The hot water temperature was measured in the bathrooms and recorded at 117.6 degrees Fahrenheit. All restrooms were observed to be stocked with an adequate supply of soap, toilet paper, and towels. LPA verified that all required Department postings were displayed and confirmed the facility was operating within its licensed capacity. Food supplies were inspected and found to include a minimum of one week of non-perishable food and two days of perishable food. Additional food storage was observed in the facility pantry and in a spare refrigerator located in the garage. Cleaning solutions, toxic chemicals, and disinfectants were observed secured under the kitchen sink and maintained in a manner that was inaccessible to residents. {***809 CONTINUE***} The facility was observed to maintain an adequate supply of clean linens. Client bedrooms were inspected and observed to have sufficient lighting to support resident safety and comfort, and each room contained the required furnishings and components. Safety equipment was evaluated during the visit. Smoke detectors were tested and found to be operational. The fire extinguisher was last inspected on January 9, 2026. Fire drills are conducted quarterly, with the most recent drill documented on February 1, 2026. The exterior of the facility was also inspected, and all passageways were observed to be free of obstructions. A shaded seating area was observed and is available for resident use. LPA reviewed two resident records and staff files and confirmed required documentation was present and current. Based on observations made during the annual inspection, no deficiencies were cited at this time. An exit interview was conducted, and a copy of this report was provided to Administrator Alexandra Gavrilas.
