Inspector’s narrative

What the inspector wrote

On today's date Licensing Program Analyst (LPA) William Vanegas conducted an unannounced visit for the purposes of conducting an annual inspection. Upon arrival LPA was greeted and granted entry to the facility by facility staff. Administrator (AD) Daryl Abarquez was notified via telephone and arrived shortly after and was able to assist LPA with the annual inspection. LPA began a tour of the facility and observed the following. AD has a valid Administrator certificate valid from June 20, 2025 through October 9, 2027. The facility is a one storied home with five bedrooms four of which are utilized for residents in care, and one of which is utilized for live in staff. The facility is equipped with an attached two car garage. LPA observed the kitchen area to be clean and free of any mildew and debris. LPA observed for the kitchen to be equipped with a gas stove, refrigerator, microwave, and a dishwasher. All appeared to be in good repair and tested operational. LPA observed a two day supply of perishable food and and a seven day supply of non-perishable food along with a sufficient amount of emergency water. LPA observed all resident rooms to be clean and free of any debris and hazards, all rooms appeared to be large enough to walk about freely and for the bedrooms to have all required furnishings including a lamp, a chair, a bed with clean linens in good repair; meaning no strains or tears, a chest of drawers, and enough storage space to store personal belongings. LPA observed all restrooms to be clean and free of any mildew and debris. LPA observed all faucets and toilets to be operational. LPA observed restrooms to have all required furnishings such as grab bars, slip resistant floor matts, and a shower chair. Hot water temperature tested between 105.4 and 107.1 Degrees Fahrenheit. CONTINUED ON LIC809-C LPA observed all smoke detectors and carbon monoxide detectors to be in good repair, and tested operational. LPA observed all fire extinguishers to be fully charged and had an updated service tag. LPA observed first aid kit to have all required items such as adhesive tape, bandages, tweezers, scissors, a thermometer, and a first aid manual. LPA toured the outside of the facility and observed the following. The backyard is large enough to participate in outdoor activities upon resident request. LPA observed for there to be an outdoor shaded sitting area. LPA observed for there to be no obstructions along the backyard areas. LPA observed side exits to be free of any obstructions and for the side gates to be unlocked and self latching. LPA reviewed two resident files, and three staff files. All files (Staff and Resident) had all required items. Annual staff training was up to date and documented correctly. LPA reviewed P&I with AD and all balances were documented correctly and corresponded to the cash on hand. LPA reviewed medications with AD per LPA review all medications are being documented correctly, and being administered per physicians orders. Based on today's observations no deficiencies will be issued per title 22 chapter 6 division 8 of the California Code of Regulations. An exit interview was conducted with AD and a copy of this report was provided to the facility.
