Pre-licensing visit·November 18, 2025

Licensing Program Analyst (LPA) Jerome Haley made an announced visit to conduct the second pre-licensing inspection. LPA Haley was greeted and granted entry by Applicant Jasmine Garcia. During a tour of the facility, LPA Haley observed that the following items have been corrected: Bed – An additional bed has been purchased and placed in the shared bedroom. Stove – Front right burner has been replaced and now lights unassisted. Auditory Exit alarms – Exit alarms have been placed on the exit doors. Exit gate – Side exit gate has springs that make the side exit gate self-closing. Excess materials – Excess materials have been removed. Postings – All postings are available on the walls and on the facility postings board. All corrections have been made, and no additional corrections are needed. Component III was presented to applicant Garcia at the end of the visit. The facility is ready to be licensed and will be recommended for licensure. An exit interview was conducted, and a copy of this report was provided.
