What the inspector wrote

Licensing Program Analyst (LPA) Samer Haddadin conducted an announced pre-licensing inspection visit (CHOW): Change Of Ownership. Upon arrival, LPA Haddadin was greeted by Administrator Troy Byington, who granted entry. Together, they toured the interior and exterior areas of the three-story facility, which includes a delayed-egress memory care unit, commercial kitchen, large dining room, medication room, multiple common areas and storage rooms, and two courtyards—one central and one dedicated to memory care—both providing shaded seating and ample space for resident activities. The facility consists of 115 resident rooms, and the census at the time of inspection was 108 residents. Resident rooms observed were furnished with a bed, chair, and clean linens, provided adequate storage space, and were free of tripping hazards. Outdoor areas, including both courtyards, were well maintained with furnishings in good repair, clear pathways, and grounds that appeared safe and inviting. All safety equipment was up to date. The fire alarm system is maintained by Fire Safety Systems, with the most recent inspection completed on March 11, 2025. Fire extinguishers were serviced by Fire Master on June 2, 2025. Hot water temperature was measured at 107.9°F, meeting regulatory requirements. The facility maintains an adequate food supply, consisting of at least a two-day supply of perishables and a seven-day supply of non-perishables. The kitchen was observed to be clean, organized, and in good repair, with all major appliances functioning properly. Cleaning supplies were securely stored in the third-floor maintenance room, ensuring they remained inaccessible to residents. A review of resident files and medication logs revealed no discrepancies. Staff personnel files were complete and contained all required documentation. Records also confirmed that the facility conducted its annual emergency drill on June 18, 2025.No deficiencies were observed during this inspection. An exit interview was conducted with Administrator Byington, and a copy of this report was provided to him at that time.
