Pre-licensing visit·September 29, 2025

Licensing Program Analyst (LPA) Samer Haddadin conducted an announced pre-licensing visit to the facility. LPA met with Licensee, Rachel Masamori, and explained the purpose of the visit. LPA toured the interior and exterior of the premises and observed the following: The facility is a one-story home consisting of six bedrooms, three bathrooms, and an attached three-car garage. At the time of the visit, there were no residents in care. The facility intends to operate as a Residential Care Facility for the Elderly. During the inspection, it was noted that the kitchen appliances were not operational, and the hot water system was not functioning. In addition, the facility did not have an infection control plan available or ready for implementation.Based on these observations, the facility is not prepared to begin operations. A follow-up appointment will be scheduled at a later date .
