Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Samer Haddadin conducted an announced visit to the facility. LPA was granted entry by Licensee Carol King and Administrator Samantha King. The purpose of the visit was to conduct a Change of Ownership inspection. At the time of the visit, the facility had six residents in care. LPA toured the interior and exterior of the facility and observed the following: The facility is a one-story home with five resident bedrooms, three bathrooms, a living room, kitchen, staff bedroom, and an attached two-car garage. During the tour, LPA observed that each resident bedroom contained the required furnishings, including appropriate bed linens, adequate closet and drawer space, and sufficient room to accommodate each resident comfortably. Toilets and water faucets were observed to be operational. Grab bars were observed to be secure, and the shower areas were observed to be clean and free of mold or mildew. Hot water temperatures were tested in all three restrooms and measured between 109.2 and 111.3 degrees Fahrenheit. LPA observed smoke detectors and carbon monoxide detectors installed in the common areas, hallways, and bedrooms. Each device was tested and found to be operational. LPA observed one fully charged fire extinguisher with a service tag reflecting the last inspection date of January 16, 2026. LPA also reviewed the facility’s fire drill documentation and confirmed that fire drills are conducted as required, with records maintained and available for review. Medication was observed to be stored in a locked cabinet. The first aid kit was stored with the medication and was found to contain all required items. The facility has a gated pool that is fenced and secured. LPA reviewed all six residents’ files and medication records, including the MARs, and found them to be current and complete. Based on LPA’s observations and record review, no deficiencies were observed pursuant to Title 22 of the California Code of Regulations. The designated Administrator was advised that final application approval will be issued by the Centralized Applications Bureau in Sacramento.
