What the inspector wrote

On November 3, 2025, Licensing Program Analyst (LPA) Garlli Tat visited the facility to conduct an announced Pre-licensing inspection. LPA was greeted and granted entry into the facility by applicants Jeanette Boyle and Vanessa Boyle. LPA observed that Jeanette Boyle has a valid Administrator certificate that expires on December 30, 2026. An initial application to operate a Residential Care Facility for the Elderly (RCFE) for a capacity of six (6) residents, five (5) of which may be non-ambulatory, one (1) may be ambulatory, and a hospice waiver has been granted for six (6), was received by Community Care Licensing (CCL). Dementia waiver has been approved. This is an initial application with one resident in care. The facility received an approved Fire Clearance on July 2, 2025 by Orange County Fire Authority. The facility is a single story home with five resident bedrooms, one shared room, a living room, a dining room, a kitchen, and an attached two car garage. On today's visit, LPA accompanied by the applicant conducted a tour of the interior portions of the facility. LPA inspected the resident bedrooms and observed them to be free of hazards. LPA observed resident bedrooms to have new furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have new, clean mattresses, linens, and blankets. LPA observed additional linens to be stored in a hallway closet. LPA inspected the two resident bathrooms and observed them to be free of hazards. Resident bathrooms were equipped with grab bars and nonskid floor mats. Faucets and toilets were operational. Hot water temperature measured between 116.9 to 118.4 degrees Fahrenheit. Continued on LIC 809-C. LPA observed the facility has a two day perishable and a seven day nonperishable food supplies on hand. LPA observed kitchen appliances such as the refrigerator, the stove, and the microwave to be clean and operational. LPA observed the five burner gas stove lights unassisted. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed chemicals and toxins to be stored and locked below the sink. LPA observed that the centrally stored medication will be kept in a separate locked in a hall storage. LPA observed a Fire Extinguisher in the kitchen and it was observed to be charged and serviced as of July 2, 2025. LPA tested the wired smoke detectors which tested operational. LPA tested the individual carbon monoxide detector which tested operational. LPA observed a land line in the kitchen. LPA observed staff and resident files kept in a locked cabinet in the resident hallway. Medications were observed to be locked in the same cabinet. LPA, accompanied by the applicant, conducted a tour of the exterior portion of the facility. LPA observed a shaded outdoor seating area with furniture for resident use. The perimeter of the facility are self-latching and can be opened in an evacuation. LPA inspected the attached garage and observed it to be kept locked at all times. LPA observed the garage to be used for storage. There are no bodies of water on the premises. Component III was completed with the applicant which provided information about how to operate the facility within compliance and reporting requirements. A follow-up inspection will be completed to address the following: -Exterior portion of the facility will be cleared of a plywood near the passageway. -The PUB 475, See Something, Say Something poster in size 20 x 26 will be mounted on the wall by the entrance. -Auditory exit alarms will be installed on resident rooms leading to the outdoors. -Applicant will have an updated facility sketch. A follow-up visit will be scheduled. An exit interview was conducted with applicant Jeanette Boyle and a copy of this report was provided.
