Pre-licensing visit·November 25, 2025

On November 25, 2025, Licensing Program Analyst (LPA) Garlli Tat conducted an announced second pre-licensing visit to ensure the facility is ready for licensure. LPA was greeted and granted entry into the facility by applicants Jeanette Boyle and Duane Boyle. Both administrators assisted with the visit. During the visit the LPA verified the following: · Applicant has an updated facility sketch indicating staff break room, capacity, and the number of bedrooms. · All auditory exit alarms are equipped and operating. · The PUB 475, See Something, Say Something poster in size 20 x 26 has been mounted by the front entrance. Exterior portion of the facility has been cleared of plywood near the passageway. With the above corrections completed, the facility’s physical plant meets the requirements of Title 22 Regulations. The applicant has met all pre-licensing requirements. LPA will submit notification to Centralized Applications Bureau (CAB) for final review prior to license being issued. An exit interview was conducted with applicant Jeanette Boyle, and a copy of this report was reviewed and provided with the applicant.
