What the inspector wrote

Licensing Program Analyst (LPA) Jenifer Tirre visited this facility for the purpose of conducting a Pre-Licensing evaluation. Facility is a single story residential home. LPA Tirre along with Licensee Machelle Thompson, Administrator Virgina Dutile, and Team Lead Almarosa Valdez toured facility at 8:42AM and observed the following: Fire clearance approval was received on 09/17/2025. Structure: Facility is a single story, 5 bedroom (4 private Residents bedrooms and 1 shared resident bedroom) 3 bathroom house with attached garage and a sand exterior. Living Room/ Dining Room : Adequate seating is available in the dining room and living room. Bedrooms Residents: All Residents bedrooms meet Licensing requirements including sufficient closet space for residents belongings. Bathrooms: All resident bathrooms have a working toilet, wash basin, and bathtub/shower as well as grab bars by toilets and non-skid mats in the shower. Linens & Hygiene Supplies: Facility has adequate supply of linens, blankets and towels. Emergency Phone Numbers and Exit Plan: Facility has Emergency Plan posted on wall in kitchen area. Food Service: Facility has 2 day perishables as well as 7 day non-perishables in the pantry/ refrigerator, as well as emergency food and water supply. Smoke Detectors: Smoke detectors/ carbon monoxide detector are centrally wired and were tested operational. Facility has 5 Fire extinguishers (4 located inside house and 1 in garage) extinguishers are mounted and fully charged. Facility has audible alarms on all exit doors and gates. Facility has two fire places that are non operational Appliances: Gas Stove and refrigerator are operational. Toxins : LPA observed cleaning supplies secured in laundry storage area and closet Water Temperature: Tested and recorded between 107.9- 113.0 degrees F. in facility bathrooms. Reading Material Games, and Equipment: facility does games, coloring supplies and activities. CONTINUED ON 809C Medications, First-Aid Kit & Book: Facility has first aid kit with required elements present at the facility. Facility has a secured location for medications and facility files. Backyard: LPA observed the facility perimeter is secured by wall with a self latching gate on both sides of facility as required. LPA observed outdoor seating. Licensee to address the following corrections by March 17, 2026: Grab bars inside showers in Restrooms 1 and 2 Side fence needs to be repaired on right side of house Monitor for computer needs to be provided or tablet device. (Facility has internet) At this time facility is not ready to be licensed. Licensee to contact LPA when corrections are complete. Component III conducted and completed during visit. An exit interview was conducted with Licensee Thompson and a copy of this report was provided.
