Interviews with staff members S1, staff member S2 and administrators indicated that staff help reposition resident R1 in bed when requested. Staff check on R1 multiple times daily and offer to help reposition when needed or requested. Interviews with S1 and S2 indicated that R1 is very particular when repositioning. S1 and S2 indicated they attempt to assist as much as possible with ADL’s but R1’s behaviors can make it difficult to navigate care at times. R1 uses a power wheelchair and is able to get out of bed and navigate around the facility with transfer assistance. R1 is considered non-ambulatory as indicated on LIC602. In review of R1’s records, R1 needs assistance with ADLs and transferring but is able to get out of bed with transfer assistance and does not require repositioning in bed. Based on records reviewed and interviews, LPA finds the above allegations to be UNFOUNDED- meaning that the allegations were false, could not have happened and/or is without reasonable basis. Exit interview conducted with the Administrator. Copy of report was given to facility.
