The fraudulent charges totaled $4,577.73. Due to these charges, R1 was unable to make their rent requirement of $1,100 per month at their current facility. LPA Parks was able to verify that these fraudulent charges were not done by any staff at R1’s current facility. The Administrator has since filed a police report due to the fraudulent charges. A complaint will be drafted for R1’s previous facility regarding staff financially abusing a resident. Based on the evidence provided, the preponderance of evidence standards was not met, therefore, the above allegation is found to be UNFOUNDED. An unfounded allegation means that the allegation was false, could not have happened and/or is without a reasonable basis. Exit interview conducted. A copy of this report was provided.
