What the inspector wrote

On 11/29/23, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit while delivering complaint findings and met with Administrator. On 11/22/23, R1 was able to leave without assistance when unsupervised and a contractor had left the front door open. R1 was found by emergency responders at a nearby park and returned to the home uninjured. R1 has dementia and is unable to leave unassisted. As a result of the investigation, the following deficiencies were cited on 809-D, per Title 22 Regulations, Division 6. (A)This poses an immediate Health and Safety risk to clients/residents in care. (B) This poses a potential Health and Safety risk, or personal rights violation, to clients/residents in care. Report reviewed. Copy of report and appeal rights provided to designee.

This requirement was not met based on reports that resident R1 left unattended before found after 50 minutes. This posed an immediate risk to the resident.
