Staff handles resident in a rough manner. LPA conducted interviews with facility staff and a facility observation was done to investigate this allegation. Interviews indicated there has been no witnessed issues with staff members handling residents in a rough manner. During LPA visit on 04/24/2025, LPA conducted a tour of the facility. LPA observed staff to be engaging with residents and when assisting them it was in an appropriate manner. During the investigation process there was no information given indicating that facility staff handled residents in a rough manner. Based on facility tour, interviews and observation, the department found this allegation is to be UNFOUNDED. Staff speaks inappropriately to resident. LPA interviewed four staff and three residents in care. LPA interviewed residents in which they stated staff do not yell at them or speak inappropriately to them. LPA interviewed staff and administrator in which they stated they have no observed staff talking inappropriately to residents. Staff interviews reflected that staff were treating all residents with respect and dignity and were not speaking inappropriately to any residents. Based on facility tour, interviews and observation, the department found this allegation is to be UNFOUNDED. Based on the investigation, the preponderance of evidence standards has not been met. Therefore, the above all allegations is found to be UNFOUNDED . A finding that the allegations are unfounded means that the allegations are false, could not have happened, and/or is without a reasonable basis. Exit meeting conducted. A copy of this report has been provided to facility.
