What the inspector wrote

Licensing Program Analyst (LPA) Sabrina Calzada arrived at the facility unannounced to conduct a Required Annual Inspection and met with staff, Joan Buchanan, who contacted Alina Costea, Licensee, who arrived to the facility shortly. Daniel Costea, Administrator, also arrived by 11:00 am. The facility is licensed for (6) non-ambulatory residents and has a hospice waiver for (2) residents. Currently, there is (1) resident, and the resident is not on hospice. LPA and Licensee toured the interior and exterior of the facility, including (6) resident bedrooms, (2) half resident bathrooms, (4) full resident bathrooms, and a staff room. Bathrooms have the necessary grab bars and non-skid flooring. . LPA observed the facility to be clean, in good repair and odor-free and each bathroom to have the necessary grab bars, non-skid flooring, and hygiene supplies. Medications are locked in a cabinet in the office, and toxins are locked in the laundry area. The inside temperature measured 76*F, and the hot water measured 108*F in the kitchen. The fire extinguisher was recently purchased in June 2025. The smoke/monoxide alarms are hardwired and operational. Sharps are locked in the pantry area. There are sufficient linens/towels/blankets and PPE supplies. Required postings are visible in the common area. LPA reviewed (1) resident and (6) staff files. Files were organized and contain current documentation. Administrator has a pending RCFE certificate- #7006238740. All staff have current First Aid/CPR certifications and have completed required annual training. An updated Dementia care information for Plan of Ops is on file and an addendum for the Admission Agreement will be added to reflect regulation changes and provide complete contact information for Ombudsman and the Department. Land line obtained. Facility to post blank copies of SOC341. There were no deficiencies observed.Exit interview. Copy of report provided.
