Post-licensing visit·December 9, 2025

Licensing Program Analyst (LPA) Melissa Parks arrived on Tuesday December 9, 2025 for an unannounced post licensing inspection. LPA met with owner Jon and explained the purpose of visit. LPA reviewed 4 resident and 1 staff files. All resident and staff files contained the required documents. LPA and Jon toured the facility together to ensure the health and safety of residents in care. The areas toured include: resident rooms, bathrooms, kitchen, garage, and backyard. In the areas toured, there were no health or safety violations observed. The following topics were discussed: resident records, staff documents, and disaster drills. Exit interview conducted. A copy of this report was provided to the Administrator.
