Pre-licensing visit·March 5, 2026

Licensing Program Analyst (LPA) Bethany Mirlohi arrived announced to complete a pre-licensing inspection. During today's inspection LPA met with Administrator Liliya Nikolaeva. Currently there are no residents at this facility. Facility was inspected both indoors and outdoors. LPA inspected 6 resident bedrooms, 2 bathrooms, garage, common living areas, kitchen, and outdoor areas. Outdoor area is free from hazardous debris. Outdoor exits are clear and accessible. First aid kit was present in the facility. Centrally stored medications will be stored in locked in kitchen cabinets. LPA inspected resident bedroom and the bedroom had appropriate furnishings, adequate lighting and storage. Bathrooms are clean, sanitary, and in good repair. Smoke detectors and carbon monoxide detectors are in working condition. Fire clearance was granted on 09/09/25 for 6 non-ambulatory residents. Kitchen is clean, sanitary, and in good repair. A working telephone has be set up for residents use. Comp III training was completed during today's inspection. Administrator agrees to inform LPA upon first attendance of client. Exit interview conducted and copy of report provided.
