Licensing Program Analyst (LPA) Sarena Keosavang arrived at the facility unannounced on 09/29/2022 to conduct a Required-1 Year Inspection utilizing the infection control domain. LPA met with facility staff, Iona Tif, and explained the purpose of the visit. Prior to initiating the annual inspection, LPA completed required COVID-19 testing protocols. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: Surgical Mask. LPA toured the facility. There are no residents present at the facility. Facility is current and active. Facility is temporarily not operating at this time. LPA notes that Licensee will maintain licensure and will update Community Care Licensing (CCL) with any changes. No deficiencies cited. Exit interview conducted.
