What the inspector wrote

This Case management – Deficiencies inspection is being conducted by Licensing Program Analysts (LPAs) Abdoulaye Zerbo and Javina George for the purpose of issuing citations for deficiencies observed during the investigation into Complaint Control No. 18-AS-20250812102840 . LPAs met with Licensee Carlos Magistrado and explained purpose of the visit. During the visit, LPAs confirmed with Licensee that an Incident report was not sent to Licensing after the event that occurred on 06-25-25. Based on observations, record review and interview, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. See LIC809-D. An exit interview was conducted and a copy of this report and appeal rights was discussed with and provided to licensee Carlos Magistrado

Licensee did not report to the Department the incident that occurred on 06-25-25 involving staff member being arresting at the facility
