Requirement for building permit before construction

Based on LPA Delgado's observation, interview and record review, the licensee did not comply with the section cited above in a 4th Bedroom was observed with a client and according to the floor plan, there is no 4th room on the floor plan which poses an immediate health, safety or personal rights risk to persons in care.

Ensure hazardous items are locked and not unattended

Based on LPA Delgado's observation and interview, the licensee did not comply with the section cited above in a canister of Clorox was observed under the kitchen sink not secured and accessible to clients in close proximity which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above due to the liability insurance on file indicating the general aggregate is $2,000,000.00 rather than a total annual aggregate of $3,000,000.00. This poses a potential health, safety and personal rights risk to persons in care.

Facility must clear fire safety for bedridden persons

Based on interview and record review, the licensee did not comply with the section cited above in [1] out of [1] residents in care who are bedridden. The facility does not have a bedridden fire clearance. This poses an immediate health, safety and personal rights risk to persons in care.

Telephone service and directory listing requirements

Based on observation and record review, the licensee did not comply with the section cited above in [2] out of [2] instances in which the facility was attempted to be contacted; however, the telephone was not in working order. On 08/11/2023 the facility was unable to be contacted by LPA, Sara Martinez, and on 08/14/2024, the facility was unable to be contacted by LPA, Stephanie Martinez. This poses a potential health, safety and personal rights risk to persons in care.

Facility must keep complete hospice care plan on file

Based on record review, the licensee did not comply with the section cited above in [1] out of [1] residents who did not have a current or complete hospice care plan on file. This which poses a potential health, safety and personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in having Staff One (S1) who was working at the facility alone have a current CPR/First aid certificate at the facility, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview and record review, the licensee did not comply with the section cited above in conducting quarterly fire and earthquake drills which poses/posed a potential health, safety or personal rights risk to persons in care.

Ensure hazardous items are locked and not unattended

Based on observation and interview, the licensee did not comply with the section cited above in keeping the cleaning solutions and disinfectants under the kitchen sink cabinet and garage inaccessible to the residents as the cabinet was unlocked and garage unlocked which poses/posed a potential health, safety or personal rights risk to persons in care.

Obtain required California clearance or exemption

Based on observation, interview, and record review, the licensee did not comply with the section cited above in having Staff One (S1) associated to work at the facility which poses/posed a potential health, safety or personal rights risk to persons in care.

Record dose details and resident medication response

Based on observation, interview and record review, the licensee did not comply with the section cited above in facility giving Resident One (R1) PRN medication ACETAMINOPHEN without documenting the date, time, and dosage that was given which poses/posed a potential health, safety or personal rights risk to persons in care.

Label and preserve medicine compliance standards

Based on observation, interview and record review, the licensee did not comply with the section cited above for R1 and their medication SIMVASTATIN 10 MG is supposed to be given to R1 in the evening per Physician's orders but on the bubble pack the facility wrote "AM". The bubble pack for SIMVASTATIN was issued on 08/01/2023 and had the 11th date popped out and dispensed which poses/posed a potential health, safety or personal rights risk to persons in care.

Food quality controls and rejected damaged goods

Based on observation, the licensee did not comply with the section cited above in having expired milk, orange juice and a corbinated drink in the facility's refrigerator which poses/posed a potential health, safety or personal rights risk to persons in care.
