What the inspector wrote

On January 20, 2023, Licensing Program Analyst (LPA) Chinwe Nwogene conducted an unannounced visit to investigate the above allegations. LPA met with Community Resource Manager, Lyssa Irani who was informed of the purpose of the visit. At the time of visit, LPA interviewed staff and residents, reviewed staff and resident file. Regarding the allegation “Facility staff are verbally abusing resident”. Interview with Resident #1 (R1) revealed staff #1 (S1) goes to resident’s dresser and when resident complaint's about it Staff #1 yells at resident. LPA interviewed staff who denied yelling at resident or being verbally abusive to resident. Staff #1 file review revealed no record of disciplinary action or record of verbally abusing resident. Regarding the allegation “Facility staff are physically abusing resident”. LPA interviewed residents who denied staff physically abuse residents. LPA interviewed staff who denied abusing residents. Regarding the allegation “Facility staff are going through resident's personal items”. It was alleged staff goes through resident's personal items in resident's dresser. LPA interviewed staff who denied going to resident’s drawer except when staff is helping resident to dress and helping get resident's clothes from resident's dresser. Based on LPA’s interview with staff, and residents and review of resident and staff file there is not enough evidence to support the above allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated at this time. An exit interview was conducted, and a copy of this report was reviewed with and provided to Lyssa Irani.
