What the inspector wrote

Licensing Program Analyst (LPA) Javier Prieto made an unannounced visit to investigate and deliver findings for the allegations listed above. LPA stated the purpose of the visit and was granted entry and met with Administrator Anna Bella Vito. The investigation consisted of a facility tour, resident interviews, staff interviews, and document review. For allegation, Resident speaks inappropriate to other residents in care: Interviews with residents and the staff revealed that there are no issues with the resident #2 (R2) speaking inappropriate to other residents. R2 was clam and alert and occasionally yell out loud due to a dementia diagnosis. For allegation, Staff not providing adequate services to resident: Interviews with residents and the staff revealed that there are no issues the services provided. The facility clean, meals are being given at the appropriate times and medications are dispensed according to doctor's orders. For allegation, Staff not able to communicate with residents: Interviews with residents and the staff revealed that there are no issues communication. Questions were answered clearly and accurately. For allegation, Facility is malodorous : Tour of the facility revealed a clean, neat facility and free of odors. For allegation, illegally eviction: Interviews with resident #1 (R1) and staff revealed that there was no eviction notice given at time of investigation. R1 left the facility cooperatively after issued of a 30 day notice. Overall, there was not enough evidence to collaborate the allegations listed above. Based on evidence obtained during the investigation, the five (5) allegations listed above are deemed UNSUBSTANTIATED. A finding that the complaints are UNSUBSTANTIATED means although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. A copy of this report was signed by LPA Prieto and Ms Vito.
