What the inspector wrote

This unannounced visit conducted by Amy Goldenberg, Licensing Program Analyst (LPA), is being conducted to initiate the 10 day visit to investigate the above-mentioned complaint allegation. This investigation included obtaining a copy of the current Liability Insurance Policy. It is alleged that the facility's liability insurance expired without payment. Review of the current policy revealed that the facility liability insurance coverage commenced on 02/01/2022. After review of the current policy, it is revealed that there was no break in liability insurance coverage. We have found the complaint allegationis unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. We have therefore dismissed the complaint. A copy of this report is being reviewed with, and furnished to the facility representative.

    Facility personnel shall at all times be sufficient in numbers, and competent to provide the services necessary to meet resident needs. Based on observation, record review and observation the facility is not meeting this requirement as evidenced by one caregiver working alone for the past 24 hours without help and providing care for residents with dementia which requiring moderate physical assistance.
