What the inspector wrote

Due to observations made by LPA Colvin, the facility was cited and deficiencies noted on LIC 9099 D. An exit interview was conducted where this report and appeal rights were discussed. A copy this report, LIC 9099D, and appeal rights were provided to Wellness Director Nathaly Ledsema during the exit interview. Regarding allegation " Facility staff not assisting residents with needs.": LPA Colvin interviewed a sample of residents and inquired about staff assistance when needed. The majority of residents able to respond confirmed that staff assist them, including with toileting. Therefore, due to lack of evidence, the allegation " Facility staff not assisting residents with needs." is UNSUBSTANTIATED. A finding of UNSUBSTANTIATED means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. It should be noted that the facility was following the care plan of both residents when the proposed incident occurred. An exit interview was conducted, and a copy of this report was reviewed with and provided to Wellness Director Nathaly Ledsema .

This requirement was not met by: Based on observation, the Licensee did not comply with the above regulation with at least two residents. R1's medications had not been administered yet, 1.5 hours after med pass, and R2's medications were not administered the previous day. This is an immediate health risk to residents in care.
