Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.

(a) If a licensee…increases the rates of fees for residents or makes increases in any of its rate structures for services, the licensee shall provide no less than 60 days' prior written notice to the residents or the residents' representatives setting forth the amount of the increase, the reason for the increase, and a general description of the additional costs… This requirement was not met as evidenced by: Based on interview and record review the facility did not issue proper notice to R1 and their representative of increase in fees. This poses a potential health saftey or personal rights risk to residents in care.
