CCL records reviewed to show that no incident report as been submitted of this injury or any other occurrence at the facility since June 2022

When the administrator is not in the facility, there shall be coverage by a designated substitute who shall have qualifications adequate to be responsible and accountable for management This was not met as evidenced by: LPA arriving to the facility with no qualified administrator for approximately 30 days.
