Licensing Program Analyst (LPA) Mary Rico conducted an unannounced visit to the facility to verify clearance of Plan of Correction from visit that was conducted on 11/16/2025. LPA Rico met with Licensee/ Administrator Marilou Panaligan and explained the purpose of the visit. The following Plan of Correction (POC)s were cleared at the time of the visit: The Licensee was cited on 11/16/2025 for Incidental Medical and Dental Care Services. During today’s visit, LPA Rico observed the facility had made corrections for all medication errors. An updated resident’s MAR was created along with the PRN. In addition, training was also provided to staff. Therefore, the POC was cleared at today's visit. The Licensee was cited on 11/16/2025 for Postural Supports. During today’s visit, LPA Rico observed the facility had made the corrections and obtained a physician order for resident’s bedrails. Therefore, the POC was cleared at today's visit. The Licensee was cited on 11/16/2025 for Maintenance and Operation. During today’s visit, LPA Rico observed the facility had made the corrections. The facility cabinets, floors, and bathroom were cleaned and organized, along with resident's personal items. Therefore, the POC was cleared at today's visit. The Licensee was cited on 11/16/2025 for Other Provisions. During today’s visit, LPA Rico observed the facility conducted an emergency fire drill and created emergency supplies readily available. In addition, the facility had cleared all emergency pathways. Therefore, the POC was cleared at today's visit. The Licensee was cited on 11/16/2025 for Hospice Care for Terminally Ill Residents. During today’s visit, LPA Rico observed the facility obtained resident’s hospice care plan for residents. Therefore, the POC was cleared at today's visit. Based on facility tour, record review and interviews, the facility Plan of Corrections has been cleared. An exit interview was conducted, and a copy of this report (LIC809) was discussed and provided to the Licensee/ Administrator Marilou Panaligan.
