What the inspector wrote

On 9/5/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced visit to the facility for the purpose of issuing citations for deficiencies observed during the investigation into Complaint Control No. 18-AS-20250826103251. LPA met with House Manager, Kailene Martinez and explained to Kailene the purpose of LPA's visit. During residents records review, LPA observed (4) four out of (5) five residents did not have a signed admissions agreement and/or admission agreement was not fully completed. Based on record review and interviews, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. See LIC809-D. An exit interview was conducted and a copy of this report and appeal rights was discussed with and provided to House Manager, Kailene Martinez..
