What the inspector wrote

Licensing Program Analyst (LPA) Raquel Hernandez attempted to conduct an annual inspection at 12:15pm at the facility. LPA knocked and rang the doorbell several times. LPA contacted and spoke with Administrator Kimberly Hamilton on the phone and she stated there are no residents living at the facility and would like to move forward with closing the facility. LPA will schedule with licensee facility closure.
