What the inspector wrote

S1 stated that all three rooms are currently in the final stages of treatment and are awaiting a final inspection by the pest control technician. LPA interviewed 5 out 5 residents and it was corroborated that they had not seen or had issues with bed bugs themselves and did not have concerns of an active bed bug infestation. Additional Witness 1 (AW1) reported that they have not personally observed any bed bugs within the facility, but became aware of the issue through informal conversations. Additional Witness 2 (AW2) stated that they observed a bed bug on the wall inside of R1’s bedroom and another on R1’s wheelchair. AW2 reported that management was informed and observed staff respond by removing the bed linens, relocating R1, and treating the room with a chemical. On August 11, 2025, LPA conducted a tour of the facility with Staff 1 (S1) and noted observations on 10 out of 10 resident bedrooms. LPA found 10 out of 10 rooms to have no evidence of active bed bugs, pests, rodents, or fly infestations. Additionally, LPA observed three additional rooms were found to be secured and under quarantine for bed bug treatment, with signage posted to restrict entry during the process. LPA confirmed through record review that a semi monthly contract is in place with pest control company Orkin. Through review of invoices, LPA noted that visits were twice a month for service on the exterior and interior perimeter of the fa cility. LPA obtained an invoice documenting a bed bug treatment conducted on June 13, 2025, which included targeted areas within the bedroom such as the bed frame, restrooms, and doorways. Based on interviews, record reviews, and observations the allegation that staff did not ensure the facility was free of bed bugs has been deemed UNSUBSTANTIATED. A finding that the allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. An exit interview was conducted. A copy of this report was provided to Executive Director Molly Bowie.
