What the inspector wrote

Licensing Program Analyst (LPA), Stephanie Martinez, conducted an announced pre-licensing inspection at the home. The LPA met with Applicant, Aurora Cuasay. There are currently five residents in care. Application : The application is for a Residential Care Facility for the Elderly, change of ownership. The fire clearance has been granted for six (6) non-ambulatory residents. Buildings and Grounds: The home is composed of four (4) resident bedrooms, two (2) bathrooms, a garage, laundry room, two (2) living room areas, two (2) dinning areas, an open kitchen, and front and back yard areas. The interior and exterior walkways of the home were observed to be clutter free with no obstructions present. Smoke and Carbon Monoxide detectors were tested and operable. There are no pools or other bodies of water located at the home. According to Cuasay, there are no weapons stored in the home. Required furniture was observed to be available in each resident room and sufficient privacy is available. The dining and living room areas are clutter free and furniture is in good condition. The resident bathrooms were observed to have non-slip mats and grab bars available. The hot water was tested and measured at 111.0 degrees Fahrenheit, which is within regulatory requirements. A washing machine and dryer are available and in working order. Outdoor areas had sufficient room for activities. Storage and Supplies: Activities were observed to be available. Medications will be stored inaccessible to any unauthorized individuals. Designated areas are available for facility files, staff files and resident files. The first aid kit was observed to be available and complete. Cleaning supplies will be stored away in a locked area. Linens, and equipment are all in good repair and sufficient for approved census. A Fire extinguisher was available and fully charged. Food Service: Utensils and dishware are sufficient for the requested capacity. The refrigerator and stove are in working order. Sharps will be stored in a locked cabinet, available only to authorized individuals. Forms: The following signs were observed to be posted at the home: Personal Rights, Resident/Family Councils, and Complaint Information. The following was observed to require follow up: The LPA observed the grass in the backyard to be completely dead and not maintained in any manner. LPA observed no shaded area in the backyard; rather, there appeared to be a remaining wood structure where a covered area used to be. The LPA also observed indoor furniture being stored outside on the patio. The furniture was not appropriately stored, as it was partially covered and found to have damage to the upholstery. The LPA observed damage to the wall in bedroom #2; The dry wall had a hole which was about four inches in width. The LPA observed spots of a rust color above the hall bathroom, shower window. The LPA observed missing light bulbs in the master bathroom and cob webs on the light fixture. LPA observed a Lightning McQueen light switch cover in bedroom #4. LPA did not observe the Theft and Loss policy to be posted. According to Applicant Cuasay, no internet accessible device is available for resident use. The LPA will inform the Centralized Applications Bureau (CAB) the home is ready for licensing once proof of corrections are received from the applicant. This report was discussed with Cuasay, and a copy was provided.
