Follow-up on corrections·December 16, 2025

On 12/16/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced visit for complaint control number 18-AS-20251212154744. During the visit, LPA met with Administrator Charles Nwapa and Charles was advised of the purpose of the visit. During the tour of the facility, LPA observed surveillance camera’s mounted on the walls in the common areas of the facility. LPA requested the facilities plan of operation to verify that the use of cameras in the common areas was established in the departments approved plan of operation. Upon request, LPA was advised by the Administrator that the facility does not maintain a copy of its approved plan of operation and LPA was not provided the plan during the duration of LPA’s visit. Therefore, a citation will be issued in accordance with Title 22, Division 6. An exit interview was conducted, and a copy of this report was provided to Administrator, Thelma Nwapa.
