What the inspector wrote

Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced case management visit to the facility. LPA met with Licensee, Jamie Pantuso and explained the purpose of the visit. LPA conducted a health and safety check by touring the facility; LPA observed (3) three of the (3) three residents present. On 6/10/2025, the department received a 3-day eviction notice regarding Resident 1 (R1). According to the information received R1 was displaying aggressive and threatening behaviors towards other residents in care, including physical posturing and verbal hostility. R1 was admitted to the facility on 6/3/2025. LPA conducted interviews with staff and residents present. Information revealed R1 has not physically attacked or harmed other residents in care however R1s outburst consisting of grunting noises will often agitate other residents in care. In addition, R1 has shown inappropriate behaviors towards staff when staff assist R1. R1 will make inappropriate sexual gestures and sexual remarks towards the staff, R1 attempted to bite staff, and R1 dug their fingernails into a staff’s arm during a behavioral episode. LPA was unable to interview the staff who experienced the behaviors of R1, however LPA was able to corroborate R1s behaviors through record review of noted incidents regarding R1, observed by staff. All other staff on shift during LPA's time of visit did not have any firsthand knowledge of the above noted incident(s). According to the Licensee, the residents responsible party and Licensee both have come to an agreement that the facility is not a good fit for R1. LPA has confirmed with Licensee that the resident was relocated on 06/13/2025. There were no health and safety concerns during the visit. Exit interview was conducted, and a copy of this report was issued, to Licensee, Jamie Pantuso.
